Key Responsibilities\n

• Collaborate closely with primary care providers to identify, treat, and manage mental health and psychosocial issues, acting as a vital member of the healthcare team.

• Conduct thorough patient intakes including diagnostic and functional evaluations; formulate treatment recommendations to meet individualized patient goals.
\n
• Provide brief and focused therapeutic interventions for clients requiring mental health services, utilizing evidence-based practices.
\n
• Advise primary care providers on which patients are suitable for primary care and which need to be referred to specialty mental health services.
\n
• Regularly follow up with patients to assess progress, making necessary adjustments to treatment plans as indicated.
\n
• Develop and implement relapse prevention plans where appropriate, ensuring patients maintain stable functioning.
\n
• Assist in identifying at-risk patients and collaborate on strategies to prevent the worsening of their conditions.
\n
• Monitor and coordinate all behavioral health services, facilitating referrals and communication with external treatment providers—always with patient consent.
\n
• Maintain detailed and accurate documentation of patient progress and treatment strategies in each client's treatment chart.
\n
• Work closely with primary care providers to formulate comprehensive treatment plans that consider all aspects of the patient's health.
\n
• Empower patients with self-management skills and educational resources, promoting active participation in their recovery journey.
\n
• Provide guidance to clinic management and team members regarding behavioral services, contributing to program evaluation and improvement initiatives.

• Support patients in managing any medical treatments prescribed by primary care providers, including coping strategies for medication side effects.
\n
• Adhere to the Code of Ethical Conduct for Clinical Professionals, maintaining the integrity and professionalism of our organization.

• Perform other responsibilities as assigned by supervisors, contributing to the dynamic nature of our clinic’s mission.

Patient Population / Client Focus\n

The LPC will engage with a diverse patient population, primarily focusing on veterans and their families facing mental health challenges. This role is crucial in addressing a wide range of issues, from PTSD and anxiety to adjustment disorders and stress management, recognizing the unique challenges faced by those who have served in the military.

Qualifications\n

• A Master’s Degree in Clinical Counseling or a related field is required.
\n
• Current Licensed Professional Counselor (LPC) license is mandatory.
\n
• Comprehensive knowledge of mental health assessment, treatment planning, developmental psychopathology, and psychotherapeutic techniques.

• Understanding of DSM-IV diagnostic criteria and effective behavioral therapy methods.
\n
• Proficiency in bilingual communication (English and Spanish) is preferred, enhancing our ability to serve a broader community.

• Strong mental and physical health to meet the demands of the position.

• Demonstrated ability to work collaboratively within a multidisciplinary team and independently.

Preferred Experience\n

Compensation and Benefits\n

This position offers a competitive salary ranging from USD 87,000 to 105,000 annually , commensurate with experience and qualifications.
