Key Responsibilities\n

• Conduct comprehensive psycho-social assessments to evaluate clients' mental, emotional, and behavioral health needs.

• Develop and implement personalized treatment plans in collaboration with clients and other healthcare professionals, ensuring that plans reflect any changes in patient status.

• Diagnose and treat a range of mental health disorders, modifying care plans as necessary to optimize therapeutic outcomes.

• Provide timely referrals to community resources, support groups, psychiatrists, and additional healthcare providers to enhance patient well-being.

• Work alongside a multidisciplinary team to create and execute effective discharge plans that consider the ongoing needs of each client.

• Educate and communicate effectively with involved parties, including patients' families, healthcare providers, insurers, and community agencies, to address challenges and promote continuity of care.

• Participate in supervision and consultation regarding discharge planning and social work interventions to mitigate potential delays in care.

• Act as an advocate for patients and families, ensuring that their needs and concerns are communicated effectively to the treatment team.

• Identify at-risk clients for delayed discharges, readmissions, or complex needs, facilitating collaboration with care providers to find optimal solutions.

Qualifications\n

• A Master’s Degree in Social Work or a related field from an accredited institution.

Compensation and Benefits\n

• Annual salary range from USD 90,000 to 100,000, commensurate with experience and qualifications.
