Key Responsibilities

• Conduct thorough physical therapy evaluations for residents, assessing their individual needs and functional capabilities.
• Create and implement personalized treatment plans aimed at improving physical function, enhancing mobility, and managing pain.
• Provide one-on-one therapy sessions focusing on mobility enhancement, strength building, balance training, and pain alleviation.
• Regularly monitor patient progress and adjust treatment plans as necessary based on their evolving needs and responses to therapy.
• Collaborate closely with nursing staff and other therapists to ensure comprehensive care and continuity in patient treatment.
• Educate patients, families, and caregivers on effective body mechanics, fall prevention strategies, and mobility techniques to empower residents in their recovery journey.
• Maintain accurate and timely documentation of therapy sessions in accordance with company policies and regulatory standards.

Patient Population / Client Focus

The primary population served by our facility includes seniors and adults undergoing rehabilitation due to various conditions. The focus is on geriatric care, where you will help patients recover from surgeries, injuries, or chronic conditions. Your proactive approach in therapy will support our residents to regain their strength, independence, and quality of life.

Qualifications

• Active Physical Therapist (PT) License in New Jersey is required.
• Completion of a degree from an accredited Physical Therapy program (DPT, MPT, or equivalent).
• Strong interpersonal and communication skills to effectively engage with patients, families, and the care team.
• Ability to work both independently and collaboratively in a team-oriented environment.
• A patient-centered approach, demonstrating compassion and empathy in all interactions.

Preferred Experience

• A minimum of 2 years of professional experience in a physical therapy role, preferably within geriatric or rehabilitation settings.
• Experience using electronic health records (EHR) systems to document patient care and treatment plans.
• Familiarity with holistic treatment approaches and therapeutic techniques that enhance mobility and quality of life for older adults.
