Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to determine patients' needs, strengths, and challenges during their end-of-life journey.

• Develop and implement individualized treatment plans that address emotional, social, and spiritual needs, utilizing evidence-based interventions.

• Maintain thorough and accurate clinical documentation in compliance with ethical standards and regulatory requirements.

• Collaborate closely with physicians, nurses, and other healthcare providers to ensure integrated care for each patient.

• Provide compassionate counseling and support sessions for patients and their families, offering education and coping strategies to deal with grief and loss.

• Utilize current research and best practices in social work to continuously improve patient outcomes and services.

Qualifications\n

Compensation and Benefits\n

The position offers a competitive annual salary ranging from $52,000 to $70,000 , commensurate with experience.
