As our Admissions Director , you'll play a vital role in helping patients and families navigate the transition into our center while building strong relationships with hospitals, referral partners, and the community. Admissions Director Responsibilities:

• Oversee all inquiries, admissions, and discharges while supporting accurate census reporting

• Coordinate the entire admissions process, ensuring all required documentation is completed accurately and on time

• Communicate with nursing, the business office, and external referral sources to coordinate admissions and bed availability

• Build and maintain relationships with hospitals, case managers, and referral partners to support census growth

• Evaluate referrals and assist with admission decisions

• Maintain accurate census, referral, admission, and discharge data

• Monitor admissions activity, referral trends, and discharge planning while providing reports as requested

• Deliver exceptional customer service to residents, families, and referral sources throughout the admissions process

Qualifications:

• High school diploma or equivalent required

• Previous admissions experience in a skilled nursing or long-term care setting preferred

• Positive, energetic, and customer-focused attitude

• Strong communication and relationship-building skills

• Ability to work independently and collaboratively

• Basic computer proficiency
