Key Responsibilities\n

• Communicate effectively with physicians and other team members to report on patient progress, emotional and behavioral needs, and any concerns or potential problems encountered during treatment.

• Maintain accurate and timely clinical records, including clinical notes and progress updates, aligned with departmental policies for documentation.
\n
• Collaborate closely with interdisciplinary health care teams to develop and implement comprehensive treatment plans, including discharge planning based on psychosocial assessments.

• Provide in-depth counseling and crisis intervention for patients facing emotional and behavioral challenges, utilizing evidence-based treatment methodologies.
\n
• Assist in facilitating discharge planning and care coordination, working harmoniously with local service providers and resources to ensure seamless access to necessary support.
\n
• Consult and communicate with family members or support systems to align their understanding and involvement in the client’s treatment and discharge processes.

Patient Population / Client Focus\n

This role primarily serves a diverse outpatient population, including individuals facing a range of mental health challenges such as anxiety, depression, trauma, and other psychological disorders. Qualifications\n

• A Master’s degree in Social Work, Mental Health Counseling, Marriage and Family Therapy, Psychology, or a related field.

• Current licensure as an LMFT, LPC, LCSW, or an equivalent mental health professional designation.

• A minimum of 3 years of experience working in an outpatient mental health setting, demonstrating proficiency in delivering effective mental health services.
