Work Environment\n

This role is structured with a focus on remote work, enabling flexibility while ensuring you can connect with clients through telehealth and remote assessments. Though primarily remote, you will occasionally travel to client homes or community locations to conduct in-person evaluations and foster relationships with clients and their families. Our standard work hours are Monday to Friday, 8:00 AM to 4:30 PM, with potential evening or weekend hours based on specific client needs.

Key Responsibilities\n

• Conduct comprehensive evaluations of clients, identifying their emotional, mental, and social needs to inform personalized care plans.

• Develop and implement individualized care strategies in collaboration with clients and their families that address specific goals, preferences, and challenges.

• Provide counseling services, offering emotional support to seniors navigating life transitions, including health changes, loss, or changes in living situations.
\n
• Advocate for clients, ensuring access to necessary community resources, services, and programs that support their needs.
\n
• Collaborate with interdisciplinary teams, consisting of nurses, caregivers, and healthcare professionals, to ensure consistent and comprehensive care delivery.
\n
• Monitor clients’ well-being through regular follow-up visits, addressing living conditions and any emerging safety concerns.
\n
• Facilitate constructive communication in family meetings, resolving conflicts and enhancing collaboration surrounding the client’s care.
\n
• Educate clients and families regarding community resources available for social engagement and support.
\n
• Maintain meticulous records of assessments, care plans, and client interactions within our electronic health record (EHR) system, upholding confidentiality and compliance with applicable regulations.

In this role, you will specifically focus on the senior client population, helping them manage challenges associated with aging and emphasizing their autonomy and participation in community life. Your expertise will enrich the lives of clients who might be facing various life transitions, including adapting to new medical diagnoses, loss of loved ones, or facing social isolation. Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Current, valid state licensure as a Licensed Clinical Social Worker (LCSW).

• At least two years of experience in social work, ideally within a community center, geriatric care, or hospice setting.

• Knowledge of social work principles, resources available for aging populations, and the challenges associated with elder care.

• Strong interpersonal skills with a demonstrated ability to foster trusting relationships with clients and families.

• Proficient in the use of electronic health record (EHR) systems and other relevant software applications to manage documentation and client records.

Preferred Experience\n

Compensation and Benefits\n

We offer a competitive salary ranging from USD 60,000 to 100,000 per year, commensurate with experience, in addition to a comprehensive benefits package.
