Work Environment\n

This role is primarily remote, allowing for flexible working arrangements that can adapt to your schedule. While you will primarily engage with clients from the comfort of your own workspace, occasional evening and weekend hours may be required to align with our patients’ needs. Our hospice center provides a unique setting where you will work cooperatively with a multidisciplinary team consisting of doctors, psychologists, and other healthcare professionals to deliver integrated patient care.

Key Responsibilities\n

• Conduct comprehensive psychosocial assessments for patients to develop individualized treatment plans aimed at enhancing their mental health and overall well-being.

• Utilize evidence-based therapeutic techniques such as solution-focused therapy, behavioral activation, and motivational interviewing to encourage positive change and adherence to treatment.

• Maintain ongoing therapeutic relationships, providing emotional support and guidance to patients and their families throughout their care journey.

• Collaborate with multidisciplinary team members to discuss treatment strategies, share insights, and ensure a coordinated approach to care delivery.

• Regularly track and evaluate patient progress towards established treatment goals using both in-person and remote communication methods, adjusting approaches as necessary for optimum outcomes.

• Assist in the management of psychotropic medications, ensuring compliance with prescribed treatments and addressing any side effects or concerns with patients promptly.

As a Licensed Clinical Social Worker at Second Chance Marriage and Family, you will primarily serve patients in a hospice setting, focusing on individuals facing terminal illnesses and their families. This role requires a high level of sensitivity and empathy, as you will help patients navigate their emotions, fears, and concerns related to end-of-life care, facilitating meaningful conversations and supporting their wishes in the journey towards comfort and peace.

Qualifications\n

• A Master's degree in Social Work from a program accredited by the Council on Social Work Education (CSWE).

• A current, valid, and unrestricted license to practice as a Licensed Clinical Social Worker (LCSW) in the State of Texas.

• A minimum of 1 year of experience as an independently licensed therapist, demonstrating your ability to provide effective therapy and mental health support.

Preferred Experience\n

While a single year of experience is the minimum requirement, candidates with experience in hospice care or with terminally ill populations are highly valued. Knowledge of family dynamics and the impact of grief on mental health is also beneficial for this role.
