Work Environment

This position is primarily onsite with a focus on home care. You will work directly with patients in their homes, necessitating a strong ability to adapt treatment plans based on their unique environments and needs. You'll be working approximately 40 hours per week, with some variability in weekend schedules. This flexibility is critical to meet the diverse schedules and needs of our patients.

Key Responsibilities

• Conduct comprehensive evaluations to assess the functional limitations and disabilities of each patient.
• Develop, implement, and modify individualized treatment plans based on clinical findings and patient goals.
• Provide educational support to patients and significant others about treatment processes and home exercise regimens.
• Maintain accurate documentation of patient progress, treatment plans, and outcomes while adhering to relevant laws and regulations.
• Collaborate effectively with physicians and fellow healthcare professionals to ensure continuity of care.
• Engage in community outreach and assist in departmental initiatives to enhance program development and patient services.
• Supervise and mentor physical therapy aides and students, facilitating a supportive learning environment.
• Participate in ongoing professional development through in-services and continuing education opportunities.

Patient Population / Client Focus

The focus will primarily be on orthopedic patients requiring rehabilitation due to post-surgical conditions, sports injuries, workplace accidents, and age-related concerns. You will interact with a diverse population, adjusting your communication and treatment approaches to meet the unique needs of patients of all ages and backgrounds.

Qualifications

• Must hold a Bachelor's Degree in Physical Therapy from an accredited institution.
• Current state licensure in Physical Therapy is required.
• Exceptional verbal and written communication skills are essential for patient education and team collaboration.
• Proficiency in computer systems for documentation and evaluation must be demonstrated.

Preferred Experience

While a minimum of 2 years of experience as a Physical Therapist is required, candidates with a background in home health care or outpatient settings will be highly valued. Experience in multidisciplinary team environments will also be considered a significant asset.

Compensation and Benefits

The salary range for this position is between $100,000 and $120,000 yearly, commensurate with experience and qualifications.
