Key Responsibilities\n

• Identify and respond to at-risk patients, creating preventive action plans tailored to their unique situations.

• Facilitate coordination of health services related to behavioral healthcare, collaborating with other providers, with patient consent.

• Maintain accurate and comprehensive documentation of patient progress, treatment plans, and diagnostic information within clinical records.
