Key Responsibilities\n

• Conducting comprehensive, ongoing assessments of clients' mental health needs to design individualized treatment plans.

• Regularly communicating with client family members, supervisors, and interdisciplinary team members to provide updates on client progress and adjust treatment as necessary.

• Designing and maintaining an organized schedule of appointments, including virtual sessions and any necessary home visits for community-based support.
\n
• Coordinating follow-up care that includes monitoring client progress through home visits, chart reviews, staff interviews, and data analysis.
\n
• Ensuring high standards of care by implementing effective treatment plans with fidelity and making necessary adjustments based on observed client needs.

• Participating actively in interdisciplinary team meetings to foster collaborative treatment strategies and share expertise.
\n
• Documenting monthly and quarterly progress notes to maintain thorough client records and compliance with legal and ethical standards required in the field.

• Organizing and leading training workshops to ensure team members are proficient in treatment methodologies and documentation standards.

• Providing thorough case management by offering follow-up support through virtual interviews and ongoing communication to facilitate client well-being.

• Acting as a liaison between clients and the broader mental health community to help fulfill individual development goals and enhance community resources.

Patient Population / Client Focus\n

As a Licensed Professional Counselor at Insight Family Therapy, you will engage with a wide-ranging demographic, including both adult and youth populations. Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a closely related field is required.
