Key Responsibilities

• Conduct comprehensive evaluations by reviewing patient histories, administering specific assessments, and developing tailored treatment plans based on evaluation outcomes.
• Communicate evaluation results and proposed treatment strategies to patients and their families, ensuring collaboration in treatment goal setting.
• Implement evidence-based treatment protocols that accommodate each patient's unique presentation and evolving healthcare standards.
• Regularly consult with a diverse team of healthcare professionals and document any significant changes in patients' conditions that might necessitate updates to their care plans.
• Facilitate thorough discharge planning, ensuring clear documentation and appropriate follow-up care. This includes maintaining accurate records of patient care activities such as evaluations, daily notes, and discharge summaries.
• Manage time effectively to meet productivity goals, maintain accurate schedules, and actively participate in departmental meetings and community marketing initiatives.
• Contribute to the establishment and achievement of departmental objectives, adhering to relevant governmental and accreditation regulations.
• Pursue ongoing education and training opportunities to refine skills, enhance competency, and fulfill organizational goals related to professional development.
