About the Role

As a  Home Health RN Case Manager , you will take the lead in planning, coordinating, and delivering skilled nursing services to clients in their homes. You’ll have the independence to manage your own caseload while being supported by a professional, collaborative team that shares your passion for excellent care.

Key Responsibilities

• Deliver skilled, compassionate nursing care according to physician orders and patient-specific plans
• Conduct in-home assessments and create individualized care goals
• Serve as a  Case Manager , collaborating with interdisciplinary team members and supervising field staff
• Provide health education and guidance to patients and their families
• Document care accurately and efficiently using our EMR system
• Maintain compliance with all clinical and regulatory standards

Qualifications

• Active  RN license in the state of Indiana
• At least 1 year of professional nursing experience ( home health experience is a plus )
• Strong critical thinking, organization, and time management skills
• Ability to work independently in the field
• Reliable transportation, valid driver’s license, and auto insurance

Job Type: Full-time

Pay: $60,000.00 - $75,000.00 per year
