Work Environment

This position is situated onsite, in a home care setting, where you will engage directly with patients in their homes. The work schedule consists of regular business hours from 9 AM to 5 PM, allowing for a balanced work-life scenario while ensuring that we meet the needs of our patients in a timely and effective manner.

Key Responsibilities

• Conduct thorough psychiatric assessments and evaluations to accurately diagnose patients' mental health conditions.
• Provide personalized medication management, monitoring the effectiveness of prescribed treatments and making necessary adjustments.
• Collaborate closely with primary care providers and other team members to offer integrated care that addresses both mental and physical health needs.
• Offer crisis intervention and support to patients during times of emotional distress, ensuring immediate care and assistance.
• Engage in community outreach and educational initiatives to promote mental health awareness and reduce stigma associated with mental health issues.
• Provide therapeutic support through individualized counseling and psychoeducation, tailored to meet the specific needs of each patient.
• Maintain accurate, confidential patient records in compliance with HIPAA guidelines and organizational policies.

Patient Population / Client Focus

You will be serving a diverse patient population that includes individuals facing various mental health challenges. Your expertise will help address conditions such as anxiety, depression, PTSD, and other behavioral health issues. Our team is dedicated to implementing trauma-informed care and cultural competence, ensuring every patient feels respected and understood.

Qualifications

• Master's degree in Nursing with a specialization as a Psychiatric Nurse Practitioner.
• Current state licensure as an Advanced Practice Registered Nurse (APRN) with psychiatric specialization.
• Strong communication and interpersonal skills to effectively interact with patients, families, and team members.

Preferred Experience

While experience in a community health center or similar healthcare setting is preferred, we value dedication and empathy in all candidates. Compensation and Benefits
