Physical Therapist – Home Health

Monday to Friday, 8-hour days.

Assignment Details

13-week contract.

Candidates will have access to a company vehicle for home visits.

Requirements

• Minimum of 1 year experience in Home Health.
• Valid Physical Therapy license.

Responsibilities

• Assess and evaluate patient needs in a home health setting.
• Implement and adjust personalized treatment plans.
• Provide education and support to patients and their families.
• Document patient progress and communicate with the healthcare team.
• Maintain compliance with healthcare regulations and standards.
