Work Environment

This position is primarily onsite, operating within the home care setting, where you will facilitate and participate in patient evaluations and treatments in their residences. We operate on a full-time schedule, typically involving 40 hours per week with variable weekends. Here, you will be an integral part of a multidisciplinary team dedicated to delivering high-quality healthcare services tailored to individual patient needs.

Key Responsibilities

• Conduct comprehensive evaluations of patients with orthopedic injuries or conditions, developing tailored treatment plans to address their individual needs.
• Implement effective treatment methodologies, incorporating a variety of therapeutic techniques to promote recovery, increase strength, and improve mobility.
• Monitor and assess patient progress, adjusting treatment plans and goals based on clinical evaluations and patient feedback to ensure the highest standard of care.
• Communicate effectively with patients, families, and healthcare providers to facilitate a seamless continuum of care, including discharge planning.
• Maintain accurate documentation of patient treatments, assessments, and progress in compliance with regulatory standards and internal protocols.
• Participate in interdisciplinary team meetings and collaborate with other healthcare professionals to enhance patient outcomes.
• Engage in community outreach initiatives and educational programs, promoting physical health awareness within the Destin area.

Patient Population / Client Focus

You will be primarily working with a diverse patient population dealing with a wide range of orthopedic issues, from post-operative rehabilitation to chronic pain management. Understanding each patient's unique journey and involving them and their families in the treatment process is fundamental to our approach, allowing for personalized care that addresses both physical and emotional recovery.

Qualifications

• A Bachelor's Degree in Physical Therapy or a doctorate in Physical Therapy is required.
• Current licensure and adherence to all state and federal regulations governing Physical Therapy practice.

• Ability to demonstrate cultural competence and provide individualized care to patients from various backgrounds.

Preferred Experience
