Key Responsibilities\n

• Conduct thorough assessments with clients to identify their goals, preferences, and available therapeutic options, ensuring a comprehensive understanding of their needs.

• Develop and implement individualized rehabilitation plans tailored to each client's educational background, skills, and aspirations, adapting these plans based on ongoing assessments and client feedback.

• Engage in close collaboration with a team of physicians, occupational therapists, and mental health professionals to create and execute rehabilitative programs that are finely tuned to meet each client's specific needs.

• Maintain detailed and accurate case files that document client history, eligibility for services, insurance information, and comprehensive narratives of all sessions.

• Regularly monitor and document client progress, adjusting treatment goals and strategies as necessary to ensure effective outcomes.

• Facilitate continuous communication with clients during their participation in job training programs and other relevant initiatives, addressing any challenges that may arise during the process.

• Work effectively with family members and other significant individuals in the client's life to enhance the support network surrounding the client.

• Complete timely analysis of client needs and prepare reports as required for case management and compliance purposes.

• Provide recommendations and referrals for additional resources and services as necessary to support client care.

Qualifications\n

• A valid Licensed Professional Counselor (LPC) licensure is mandatory.
