Key Responsibilities\n

• Conduct thorough evaluations and assessments to identify the psychosocial needs of clients, developing individualized care plans in accordance with their unique circumstances.

• Collaborate with interdisciplinary team members to develop and implement individualized family service plans (IFSP), ensuring all components work toward optimal client outcomes.

• Regularly monitor and assess client progress, adjusting services as necessary to meet their evolving needs.
\n
• Provide emotional support to families, helping them understand the challenges faced by their loved ones and offering resources to enhance coping strategies.

• Adhere strictly to all Federal and State regulations for social work practice, ensuring ethical standards are upheld at all times.

• Establish strong relationships with clients and their families, facilitating trust and open communication to promote a healthy therapeutic environment.

Patient Population / Client Focus\n

The primary clientele for this role includes seniors receiving hospice care. Our focus is on vulnerable populations who may be dealing with terminal illness, cognitive decline, and the accompanying emotional challenges of end-of-life care. As a Social Worker, you will help families navigate grief and loss while advocating for their loved ones' psychological well-being.

Qualifications\n

• A Master's degree in Social Work (MSW) from an accredited program is required.

• Current Licensure as a Clinical Social Worker (LCSW) in the state of Texas.
\n
• Expertise in assessment, diagnosis, and treatment approaches for older adults, as well as crisis management and risk assessment.

• Strong verbal and written communication skills to effectively interact with clients, families, and team members.
\n
• Demonstrated organizational skills and attention to detail in case documentation and reporting.

• Cultural competency in addressing specialty issues related to senior populations and their unique developmental needs.

Preferred Experience\n

• A minimum of two years of experience in clinical social work, particularly in hospice or geriatric settings, is preferred.

Compensation and Benefits\n
