Director of Social Work

Director of Social Work Silver Stream Rehabilitation and Nursing Center – Full-Time Silver Stream Rehabilitation and Nursing Center is seeking a compassionate and experienced Director of Social Work to lead our Social Services Department. The ideal candidate will be passionate about resident advocacy, family support, discharge planning, and ensuring the psychosocial well-being of our residents. Responsibilities: Direct and oversee the facility's Social Services Department Complete psychosocial assessments and develop individualized care plans Assist residents and families with adjustment to the facility and ongoing psychosocial needs Coordinate discharge planning and community resource referrals Advocate for resident rights and address resident and family concerns Participate in interdisciplinary care plan meetings Ensure compliance with federal, state, and facility regulations Collaborate with nursing, admissions, therapy, and administrative staff to promote positive resident outcomes Maintain accurate and timely documentation Qualifications: Bachelor's Degree in Social Work required; Master's Degree preferred Pennsylvania Social Work license (LSW) preferred Previous experience in a skilled nursing, rehabilitation, or long-term care setting preferred Knowledge of discharge planning, resident rights, and long-term care regulations Strong communication, leadership, and organizational skills Compassionate and resident-centered approach to care Benefits: Competitive salary Health, dental, and vision insurance Paid time off and holiday pay Professional development opportunities Supportive and collaborative work environment Join a team committed to providing exceptional care and support to residents and their families.
