Key Responsibilities\n

• Coordinate with Reentry Social Workers to deliver tailored discharge planning services, ensuring continuity of care for patients transitioning out of institutionalized settings.

• Complete required statistical reports to monitor patient progress and outcomes, supporting our commitment to excellence in care.
\n
• Collaborate effectively with the Department of Correction to facilitate access to treatment, safe housing options, and appropriate care disposition for incarcerated individuals.
\n
• Develop and lead group therapy sessions within mental health observation units, focusing on fostering connections and promoting mental wellness among participants.
\n
• Maintain meticulous documentation of patient interactions and therapeutic interventions, while participating in weekly clinical supervision with the Clinical Supervisor.
\n
• Engage in mental health and medical rounds, contributing insights on patient care and treatment strategies during multidisciplinary meetings.
\n
• Actively participate in case conferences and treatment planning for patients receiving mental health services, ensuring a holistic approach to care.
\n
• Conduct mental health evaluations, crisis intervention assessments, and provide follow-up care to ensure patient stability and satisfaction.

• Utilize psychological testing methodologies as assigned to assess patient needs and tailor interventions accordingly.

Compensation and Benefits\n
