Position Details Registered Nurse (RN) – Home Health for Graham Healthcare Group, Emigsville, Pennsylvania. Specialty: Home Health Discipline: RN Employment Type: Staff Duration: Ongoing Hours: 40 hours per week (8 hour shifts) Responsibilities Lead and collaborate with care teams to deliver management and delivery of patient care plans. Provide patient care to individuals in York, PA and surrounding areas. Document clinical activities, consistently meeting critical deadlines for charting. Manage time and schedule independently, including traveling approximately 20% of the day to patient locations. Qualifications Active RN license in the state of employment. Graduate from an approved practical nursing school. Minimum one year of RN experience with clinical competence in patient care. Valid driver’s license and ability to drive to patient locations.
