Key Responsibilities\n

As an LCSW at Relationship Behavioral Health Services, your responsibilities will include:

• Conducting thorough assessments to determine the counseling and support needs of clients experiencing co-occurring disorders.

• Utilizing DSM (Diagnostic and Statistical Manual) criteria to diagnose substance use issues and recommend appropriate levels of care as per ASAM (American Society of Addiction Medicine) guidelines.

• Preparing and implementing the substance abuse counseling component of individualized treatment plans tailored to each client's unique needs.

• Collaborating with a multidisciplinary team to develop discharge and continuum of care plans that facilitate clients’ transitions to the appropriate levels of treatment post-detox.

• Meticulously documenting all counseling services, assessments, referrals, and follow-ups in clients' clinical records to ensure continuity and quality of care.

• Managing a caseload that includes case management activities and clinical updates, ensuring comprehensive tracking of each client’s progress.

• Coordinating with external support systems, including legal representatives, educational institutions, and employers, to provide holistic support to clients and their families.
