Full-Time Physical Therapist - Home Care Setting

Echo Rehabilitation Center, a premier provider of physical therapy services, is seeking a dedicated and skilled Physical Therapist for our home care division in Port Jervis, New York. Work Environment

As a Physical Therapist in our home care setting, you will primarily work in patients' residences, allowing for a personalized touch in therapy delivery. This setting enables you to evaluate how patients manage their day-to-day activities and tailor interventions to fit their home environment. Collaboration with multidisciplinary teams, including occupational therapists and nurses, is essential to provide cohesive care that focuses on holistic healing.

Key Responsibilities

• Conduct detailed assessments of each patient's physical condition, utilizing evidence-based practices to devise individualized treatment strategies.
• Administer various therapeutic interventions, including manual therapy techniques, resistance exercises, and proprioceptive neuromuscular facilitation (PNF), aimed at enhancing mobility and patient independence.
• Establish, document, and review measurable patient goals, regularly adjusting treatment plans based on progress and patient feedback.
• Educate patients and their families on therapeutic exercises, injury prevention techniques, and wellness strategies to foster self-management of health conditions.
• Maintain accurate and timely patient records and documentation as mandated by state regulations, ensuring compliance with healthcare standards and practices.
• Work collaboratively with healthcare professionals to update care strategies and address patient concerns in a timely manner.
• Participate in ongoing training and development to stay current with industry advancements and best practices in physical therapy.

Patient Population / Client Focus

Your primary focus will be on adult and geriatric patients who may be recovering from surgeries, managing chronic conditions, or regaining strength after illness or injury. By understanding each patient's personal challenges and goals, you will create manageable and realistic rehabilitation paths, ultimately improving their quality of life.

Qualifications

• Possession of a current New York State Physical Therapist license.
• Completion of a Bachelor's Degree in Physical Therapy or related field; a Master's (MSPT) or Doctorate (DPT) degree is preferred.
• A minimum of two years of clinical experience in physical therapy, ideally within home care or outpatient rehabilitation settings.

Preferred Experience

• Experience in developing and implementing home exercise programs tailored to rehabilitation goals.
• Proficiency in the use of physical therapy modalities and equipment, including therapeutic exercise bicycles and resistance bands.
• Familiarity with electronic health record (EHR) systems for documentation and patient management.

Compensation and Benefits

This position offers a competitive salary ranging from $100,000 to $120,000 annually, depending on experience and qualifications, accompanied by a comprehensive benefits package that includes:
