Work Environment

This position is based onsite as part of our home care services in Port Jervis, NY, allowing you to engage directly with patients in the comfort of their homes. You will be part of a supportive team, including nurses, occupational therapists, and therapy aides, who collectively ensure holistic patient care. Key Responsibilities

• Evaluate and assess patients' physical conditions, creating individualized treatment plans based on tailored goals and recovery timelines.
• Deliver hands-on therapeutic techniques, including manual therapy, therapeutic exercise, and neuromuscular re-education to improve function and mobility.
• Document all patient interactions and treatment sessions accurately in our health management system to maintain a comprehensive patient history.
• Communicate effectively with patients and their families, providing clear instructions, valuable information, and addressing any concerns they might have regarding their care.
• Monitor progress and modify treatment plans according to patient responses and rehabilitation goals.
• Manage a daily patient caseload of approximately 7 patients, ensuring each receives thorough and timely treatment during visits.

Patient Population / Client Focus

Your primary focus will be on patients recovering from various conditions, including orthopedic injuries, surgical recovery, neurological disorders, and chronic pain management. By working closely with each patient, you will develop a deep understanding of their individual needs, preferences, and lifestyle factors that impact their recovery. Building trust and rapport is crucial for successful rehabilitation outcomes, and you will be empowered to make a positive difference in their lives.

Qualifications

• Valid New York State Physical Therapist License.
• Bachelor's degree in Physical Therapy; a Master of Science in Physical Therapy (MSPT) or Doctor of Physical Therapy (DPT) is preferred.
• A minimum of two years of relevant clinical experience in rehabilitation settings is required.

Preferred Experience

• Experience in home care services and familiarity with community-based rehabilitation practices.
