Key Responsibilities\n

As a Licensed Clinical Social Worker, your responsibilities will include:

• Conducting thorough psychosocial assessments to understand patients' emotional, social, and financial needs.

• Providing individual and group counseling focused on adjustment issues, grief, coping strategies, and emotional resilience during challenging health transitions.

• Collaborating closely with medical teams to develop personalized care plans that address the holistic needs of our patients.

• Coordinating post-discharge services by engaging with community organizations to facilitate smooth transitions in care.

• Offering education and emotional support regarding treatment options, navigating complex healthcare decisions, and available resources.

• Actively participating in hospital committees to advocate for social work-related issues and improve organizational practices.

• Providing crisis intervention as needed, ensuring patients and their families receive timely support during stressful situations.

• Maintaining accurate and compliant documentation that meets hospital policies and legal requirements.

Patient Population / Client Focus\n

You will be primarily working with hospice patients and their families, focusing on emotional support as they navigate serious or terminal illnesses. This role requires a deep understanding of the unique challenges faced by patients and their loved ones during end-of-life care, emphasizing the importance of compassion, dignity, and respect in every interaction.

Qualifications\n

A successful candidate for the Licensed Clinical Social Worker position will hold:

• A Master's degree in Social Work from an accredited program.

• Current and active state licensure as a Licensed Clinical Social Worker (LCSW).
