Position Summary Seeking a Clinical Coordinator III (RN) to work within the outpatient practice of the Center For Advanced Cardiac Care (CACC), Advanced Heart Failure, LVAD and Cardiac Transplantation program, under the leadership of the Chief Nurse Practitioner and direction of the CACC Medical Directors. The Clinical Coordinator III (RN) will be directly supervised by the Chief Nurse Practitioner. The Clinical Coordinator III (RN) will work collaboratively with Physicians, Nurse Practitioners, other members of multidisciplinary care team, and administrative support staff unto the CACC Program. Responsibilities Provide comprehensive patient care coordination and patient/caregiver education. Support the ongoing clinical operations of the CACC Advanced Heart Failure, LVAD, and Cardiac Transplant Program to ensure delivery of compassionate, patient-centered care. Coordinate diagnostic testing, outpatient clinic follow-up, and review of diagnostic reports. Participate actively in the day-to-day management of patients and support the strategic vision of the CACC program. Implement nursing care in the outpatient setting in collaboration with Advanced Practice Providers (APPs) and physicians, for the center of the CACC. Provide clinical support and patient care management within an outpatient clinical and administrative setting. Manage ambulatory patients with advanced heart failure, LVADs, and pre- and post-heart transplantation needs across the continuum of care. Monitor patients receiving oral, intravenous, and inhaled therapies and coordinate appropriate follow-up care. Obtain, review, and communicate laboratory and diagnostic test results to the care team and patients as appropriate. Evaluate symptomatic patient calls and perform telephone triage within Clinical Coordinator III (RN)’s scope of practice. Provide initial and ongoing patient and caregiver education regarding medications, treatment plans, dietary recommendations, lifestyle modifications, and disease management. Administer nursing care in the clinic as directed by their collaborating Advanced Practice Providers (APPs) and physicians. Complete accurate and timely documentation in the electronic medical record (Epic). Provide remote patient monitoring and assessment in collaboration with supervising providers. Demonstrate flexibility, critical thinking, and effective communication skills in managing complex patient needs. Adhere to all applicable HIPAA, billing compliance, Joint Commission, institutional, and patient safety standards. Participate in quality improvement initiatives and multidisciplinary team activities. Perform additional duties as assigned appropriate to scope, education, and/or training. Minimum Qualifications Bachelor's degree in Nursing (BSN) or an equivalent combination of education and experience. Minimum of four (4) years of nursing experience in a healthcare setting. Current New York State Registered Nurse (RN) license. Must be eligible for Basic Life Support (BLS) certification. Preferred Qualifications Experience in advanced heart failure, ventricular assist device (LVAD), cardiac transplantation, cardiology, cardiothoracic surgery, intensive care, coronary care, telemetry, step-down, or emergency department settings. Ambulatory care and patient care coordination experience. Current Advanced Cardiac Life Support (ACLS) certification preferred. Experience with Epic electronic medical record systems. Other Requirements Proficiency in Microsoft Office Suite. Strong interpersonal, written, and verbal communication skills. Ability to work effectively in a multidisciplinary healthcare environment. Demonstrated organizational skills and ability to manage multiple priorities in a fast-paced clinical setting. Ability to exercise sound clinical judgment and maintain professional accountability. Commitment to patient-centered care, quality improvement, and ongoing professional development. Ability to maintain confidentiality and comply with HIPAA and institutional policies. Demonstrated professionalism in all internal and external interactions. Completion of all required institutional, compliance, and systems training. Participation in medical surveillance program is required: Contact with patients and/or human research subjects Wear N-95 respirator Potential bloodborne pathogen exposure Successful completion of applicable compliance and systems training requirements.
