Work Environment

This role is primarily onsite, working directly in patients' homes throughout the Centreville area. As part of our home care initiative, you will engage with patients in their familiar surroundings, enabling a personal touch that aids in their recovery. Key Responsibilities

• Conduct comprehensive evaluations of patients to assess their physical condition and identify specific treatment needs.
• Design and implement individualized treatment plans utilizing diverse techniques including therapeutic exercises, manual therapy, and specialized equipment to alleviate pain and improve functionality.
• Monitor and document patient progress consistently, adjusting treatment protocols based on clinical judgment and patient feedback.
• Educate patients and family members on proper therapeutic techniques, exercises, and the use of assistive devices, ensuring they are equipped for continued recovery outside of sessions.
• Maintain clear communication with an interdisciplinary team, collaborating with physicians and other healthcare professionals to optimize patient care.
• Stay informed on the latest advancements in physical therapy through ongoing education, and participate in workshops to enhance professional skills.

Patient Population / Client Focus

You will be working with a wide range of patients, including children and adults with varying diagnoses. Your role will be critical in helping individuals regain independence after surgery, injury, or illness. By delivering one-on-one care in their homes, you can build trusting relationships that foster therapeutic alliances, allowing for tailored approaches to rehabilitation.

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited institution.
• Current and valid license to practice Physical Therapy in Virginia.
• Strong written and verbal communication skills, alongside proficiency in computer software for documentation and reporting.
• Ability to demonstrate flexibility, agility, and a patient-centered approach in your care.

Preferred Experience

While a minimum of 2 years in a physical therapy role is required, we are open to exceptional new graduates who bring a keen enthusiasm for home health care and a desire for professional growth. Experience in a home care setting is a plus.

Compensation and Benefits
