Key Responsibilities\n

• Conduct thorough assessments of clients’ social, emotional, and psychological needs, utilizing structured interview techniques and standardized assessment tools.

• Provide tailored individual and group counseling, employing evidence-based therapeutic methods such as Cognitive Behavioral Therapy (CBT) and Trauma-Informed Care.

• Develop, implement, and regularly review individualized service plans that align with clients' goals, leveraging best practices within hospice-related mental health support.

• Act as a strong advocate for client rights, ensuring access to necessary resources and addressing relevant social justice issues through collaboration with community partners.

• Document progress on treatment plans and service delivery efficiently in electronic health record systems, ensuring compliance with regulatory standards and agency protocols.

• Play a vital role in program evaluation and grant reporting, contributing valuable insights based on client feedback and engagement outcomes.

• Engage in regular team meetings and professional development sessions, sharing knowledge and strategies to enhance overall client care and organizational impact.

Patient Population / Client Focus\n

In this role, you will primarily work with individuals and families navigating the complexities associated with end-of-life care, bereavement, and chronic illness. Qualifications\n

• Master's degree in Social Work (MSW) from an accredited institution.

Compensation and Benefits\n
