Key Responsibilities\n

• Conduct comprehensive assessments for patients to evaluate their mental health needs and develop individualized treatment plans.
\n
• Actively communicate with multidisciplinary team members, families, guardians, and caregivers to ensure coordinated care and to share updates on patient progress.
\n
• Schedule and provide home visits, and document insights that may inform the ongoing care and support of individuals.
\n
• Facilitate and engage in Interdisciplinary Team Meetings, contributing to discussions surrounding patient care strategies and treatment effectiveness.
\n
• Maintain meticulous records and documentation, including monthly and quarterly progress notes, to uphold compliance with regulatory standards.
\n
• Serve as a liaison between the healthcare team and the mental health community, ensuring that community resources are appropriately utilized.
\n
• Provide training and support to clinical staff and team members to enhance their competencies in carrying out treatment plans and adhering to documentation protocols.
\n
• Perform follow-up evaluations, home visits, and assessments to continually address the needs of individuals receiving care.

Patient Population / Client Focus\n

You will work with a diverse array of clients, including adults and youth experiencing various mental health challenges. Special interest in areas such as family therapy, couples therapy, trauma-informed care, or substance misuse will guide your practice, enabling you to tailor interventions that resonate with each individual's unique circumstances. This role demands sensitivity, understanding, and the ability to build rapport with a wide range of clients from different backgrounds.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related discipline is required.
\n
• Active and unrestricted licensure as a Professional Counselor (LPC) in the State of Oregon.
\n
• A minimum of two years of experience in a clinical counseling role is essential;

• Demonstrated knowledge of legal and ethical standards in mental health practice is required.

• Strong interpersonal and communication skills necessary for effective collaboration and client engagement.

• Proficiency in electronic medical record (EMR) systems and related technologies.

Preferred Experience\n

• Experience in specialized fields such as family therapy, couples therapy, trauma, or substance misuse will be highly regarded.

• Annual salary range of $90,000 – $100,000, commensurate with experience and caseload.
