Work Environment

This position is primarily home-based, providing therapy sessions in the patients' residences. Our hands-on approach allows you to create an intimate healing environment in which you can observe and tailor your treatments to the home setting, fostering a deeper connection with your clients. Working onsite means you will be on the front lines, directly interacting with patients and understanding their home contexts, which is crucial for effective rehabilitation.

Key Responsibilities

• Evaluate patients by performing comprehensive assessments and determining their physical therapy needs and rehabilitation goals based on referrals from physicians.
• Develop, implement, and adjust individualized treatment plans that align with best practices and the latest research in physical therapy.
• Utilize evidence-based practices to treat a variety of conditions, focusing on orthopedic, pediatric, neurologic, and sports medicine specialties.
• Deliver purposeful therapeutic care at patients' homes, employing a range of techniques, including manual therapy, therapeutic exercises, and patient education.
• Monitor patient progress, document treatment outcomes, and communicate effectively with multidisciplinary teams to optimize patient care.
• Provide extensive education to patients and their caregivers, empowering them with strategies and knowledge for ongoing rehabilitation success.
• Actively participate in professional development activities while meeting utilization goals and contributing to overall business objectives.

Patient Population / Client Focus

Your work will encompass a diverse patient population, with a focus on individuals recovering from surgeries, injuries, or chronic conditions. You will have the satisfaction of helping patients from various backgrounds achieve their mobility and functional goals while maintaining their independence in the comfort of their homes.

Qualifications

• A Doctorate degree in Physical Therapy (DPT) from an accredited institution.
• A valid state license as a Doctor of Physical Therapy (DPT) as required by the Physical Therapy Examining Committee.
• Minimum of two (2) years of professional physical therapy experience.
• Current CPR certification demonstrating commitment to patient safety.
• Excellent communication skills, both written and verbal, for effective patient interaction and team collaboration.
• Proficiency in computer software necessary for documentation and billing, including word processing and therapy software.
• Strong interpersonal skills to build robust, trustworthy relationships with patients and their families.

Preferred Experience

Although not mandatory, proficiency in using advanced therapeutic modalities and techniques in a home care setting is highly desirable. Familiarity with telehealth practices is an advantage as we continue to expand our service delivery methods. Previous experience in a similar role involving home care would be beneficial for adapting rapidly to the unique challenges of this setting.

Compensation and Benefits

This position offers a competitive salary ranging from $100,000 to $120,000 annually, commensurate with experience.
