As a Care Partner at Highgate at Bellingham, you will provide exceptional care and support to our residents, ensuring their physical, emotional, and social needs are met. You will work closely with our nursing team to develop personalized care plans, assist with daily living activities, and create a warm and welcoming environment for our residents.

Responsibilities

• Provide one-on-one support to residents with daily living activities, such as bathing, dressing, and grooming

• Assist with medication management, vital signs, and other health-related tasks

• Develop and implement individualized care plans with the nursing team

• Promote a positive and respectful environment, fostering a sense of community among residents and staff

• Communicate effectively with residents, families, and healthcare professionals to ensure seamless care coordination
