Job Details:

• Title: Physical Therapist (PT)
• Employment Type: Full -Time | Permanent
• Location: Piscataway, NJ 08854
• Schedule: 8:30 AM – 4:30 PM
• Pay: $48-$55/hr. (Based on experience) + Benefits

• Skilled Nursing Facility (SNF)
• Post-Acute Rehabilitation

-> Responsibilities

• Evaluate patients and develop individualized physical therapy treatment plans.
• Provide skilled PT interventions to improve mobility, strength, balance, coordination, and functional independence.
• Treat patients with orthopedic, neurological, and medically complex conditions.
• Monitor patient progress and modify treatment plans as needed.
• Collaborate with physicians, nurses, occupational therapists, speech therapists, and interdisciplinary care teams.
• Complete accurate and timely clinical documentation.
• Educate patients and caregivers on therapy goals, safety techniques, and home exercise programs.
• Participate in interdisciplinary care planning and maintain compliance with Medicare and state regulations.

-> Qualifications

• Active New Jersey Physical Therapist (PT) license .
• Graduate from an accredited Physical Therapy program (DPT preferred).
• Experience in Skilled Nursing Facility (SNF), Long-Term Care (LTC), Rehab, or Post-Acute Care preferred.
• Strong communication and documentation skills.
• CPR/BLS certification preferred.
