Key Responsibilities\n

In this role, your day-to-day responsibilities will include:

• Providing clinical behavioral health services that reflect our health center’s mission of improving patient care and health outcomes.

• Assisting mental health consumers in navigating the complexities of both physical and behavioral health care systems, ensuring they receive integrated and effective care.

• Educating patients about available resources and services to encourage active participation in their health, including disease prevention and maintenance of a healthy lifestyle.
\n
• Actively participating in multi-disciplinary treatment team meetings to discuss strategies, share insights, and coordinate care.
\n
• Reporting all significant incidents, such as crises, to the Medical Director, and ensuring immediate communication of psychiatric and medical emergencies to the Chief Medical Officer.
\n
• Completing thorough documentation of clinical encounters in adherence to CHDC’s standards and regulatory requirements.
\n
• Contributing to our Quality Improvement Program by participating in meetings and training sessions aimed at enhancing service delivery.
\n
• Utilizing evidence-based screening tools to assess the healthcare needs of individuals and appropriately reporting findings.
\n
• Collaborating with the Center of Excellence Team to support patients with Opioid Use Disorder, providing necessary interventions as part of an integrated treatment approach.

Patient Population / Client Focus\n

As a Licensed Clinical Social Worker at CHDC, you will engage with a diverse population of patients, including those struggling with mental health issues, substance use disorders, and general health needs. Your work will focus on patient-centered approaches that respect the individuality of each client while addressing the socioeconomic factors affecting their well-being. Through this role, you play a critical part in promoting mental health awareness and access to resources for underserved communities in Pennsylvania.

Qualifications\n

• A valid Licensed Clinical Social Worker (LCSW) credential.

Compensation and Benefits\n
