This remote role allows you to work from the comfort of your home while supporting residents of our hospice center located in Manheim, Pennsylvania. Key Responsibilities\n

• Conduct thorough assessments of patients’ psychosocial needs and develop tailored treatment plans that prioritize their mental health and emotional well-being.

• Provide individual, family, couple, and group psychotherapy sessions, utilizing evidence-based practices and therapeutic techniques appropriate for hospice care.

• Collaborate with the psychiatry team and other healthcare professionals to plan and coordinate treatment strategies, discussing patient progress and updating care plans as needed.

• Evaluate and document the ongoing biopsychosocial needs of patients, identifying potential barriers to treatment and tailoring approaches to meet those challenges.

• Manage caseload responsibility effectively, ensuring continuity of care and comprehensive support for assigned patients, alongside providing input and feedback regarding patient care in the program.

Patient Population / Client Focus\n

As a Licensed Clinical Social Worker in our hospice center, you will primarily work with elderly patients facing chronic illness and end-of-life transitions. Qualifications\n

• A Medicare provider number is preferred, although not mandatory.

Compensation and Benefits\n

The position offers a competitive salary range of $90,000 to $100,000 annually, commensurate with experience.
