Key Responsibilities\n

• Conduct thorough clinical social work evaluations, employing both quantitative and qualitative methodologies to assess patient needs.

• Engage clients in the development of personalized aftercare plans, outlining clear goals and resources that aim to sustain their recovery post-discharge.

• Closely monitor medication side effects in collaboration with the Psychiatrist, ensuring feedback and reports are documented and acted upon.

• Facilitate crisis intervention as necessary, employing active listening and conflict resolution skills to assist clients facing urgent needs.

• Oversee and maintain a manageable caseload effectively, ensuring all required documentation is completed in a timely manner according to company policies and best practices.

• Participate proactively in treatment team meetings and case management discussions, contributing valuable insights to care coordination efforts.

• Document detailed assessments, treatment plans, goal setting, progress notes, and discharge planning to ensure compliance and reflect comprehensive patient care.

• Research and refer clients to essential community resources, advocating for services such as food assistance, childcare, and healthcare to enhance their support network.

Patient Population / Client Focus\n

In this role, you will work primarily with individuals experiencing substance use disorders who are navigating the challenges of detoxification. Our patient base is diverse, requiring sensitivity to cultural differences, trauma-informed care approaches, and tailored therapeutic interventions that address the complex psychosocial factors affecting each individual’s journey to recovery.

Qualifications\n

• A Master’s Degree in Social Work from an accredited program is required.

• Must possess a valid California Clinical Social Worker License (LCSW), active and unrestricted.

• A minimum of 2 years experience in a behavioral health or clinical setting, preferably in case management or social work.

Compensation and Benefits\n
