Hospice RN Case Manager | Home Health + Travel
OPTIMAL CARE INC
Optimal Care is seeking a Registered Nurse to provide skilled home health and hospice care across Grand Rapids and surrounding counties. You’ll manage a patient caseload, perform comprehensive assessments, develop personalized care plans, and coordinate with physicians, families, and an interdisciplinary team to deliver high-quality care. This role requires travel to patients' homes and facilities, with on-call rotation on weekends. #J-18808-Ljbffr
