Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to evaluate clients' emotional and psychological needs, identifying both strengths and areas requiring support.

• Facilitate individual, family, and group therapy sessions, customizing interventions based on the unique needs and cultural backgrounds of clients.

• Utilize evidence-based therapies, including cognitive-behavioral therapy and motivational interviewing, to address clients’ specific mental health challenges effectively.

• Develop and document detailed treatment plans, setting measurable goals and tracking progress regularly to ensure accountability and success in treatment.

• Collaborate closely with a multidisciplinary team, including psychiatrists and medical staff, to create a holistic care approach that meets all facets of client needs.

• Deliver crisis intervention services, including comprehensive risk assessments and safety planning, to ensure the immediate well-being of clients.

• Serve as an advocate for clients, helping them navigate available community resources and providing essential case management support.

• Engage in outreach initiatives to educate the community about mental health issues, actively participating in events that promote mental wellness and reduce stigma.

Patient Population / Client Focus\n

You will work with a diverse clientele, often including individuals and families dealing with terminal illness and end-of-life matters. Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required.

• Active licensure as a Licensed Clinical Social Worker (LCSW) in the state of New York.
