Key Responsibilities\n

• Conduct thorough psychosocial assessments to identify patients’ mental health needs and create individualized treatment plans that reflect their unique circumstances.

• Coordinate care with physicians, nursing staff, and community resources, ensuring a seamless continuum of care.

• Provide evidence-based psychotherapeutic treatment (individual, family, or group therapy) and monitor patients' progress in aligned with health-related goals.

• Document and chart all patient interactions in compliance with established Medical Records policies, ensuring accurate and up-to-date patient files.

• Act as a liaison between community agencies and the hospice center, facilitating additional resources for patients and ensuring they receive comprehensive support.

• Advocate for patients during disposition planning, empowering them and their families to make informed choices about their care.

• Maintain an up-to-date knowledge of social work ethics, state regulations, and community resources relevant to patient needs.

Patient Population / Client Focus\n

In this role, you will primarily serve patients receiving hospice care, often dealing with advanced illnesses and end-of-life issues. Qualifications\n

• A valid Licensed Clinical Social Worker (LCSW) license with no disciplinary actions.

• A Master’s Degree in Social Work (MSW) from an accredited institution.

Compensation and Benefits\n

The annual salary for this position ranges between USD 120,000–130,000, commensurate with experience.
