Key Responsibilities\n

• Conduct comprehensive intake assessments and psychosocial evaluations to determine the emotional, social, and environmental factors affecting the patient’s well-being.

• Develop, present, and obtain approval for individualized treatment plans during Clinical Development and Review Process (CDRP) and treatment team meetings, ensuring alignment with the best practices in addiction recovery.

• Engage in clinical case conferences to discuss patient progress, treatment adjustments, and collaborative strategies to enhance client outcomes.

• Lead educational sessions focused on chemical dependency, ensuring patients and families understand the complexities of addiction and recovery paths.

• Provide diverse counseling services, encompassing individual and group therapy, as well as specialized sessions for adolescents, significant others, families, and multi-family groups, where applicable.

• Deliver case management services by coordinating resources, referrals, and support systems to empower patients and enhance their recovery journey.

Patient Population / Client Focus\n

This role has a unique emphasis on hospice care, where staff members must demonstrate sensitivity and understanding towards patients facing life-limiting illnesses, often coupled with mental health struggles such as anxiety, depression, and grief. You will work closely with families to help them navigate these challenges, providing emotional support that addresses both the individual and family unit.

Qualifications\n

• A Master's degree in Social Work (MSW) or a related field is required.

• Current licensure as a Licensed Independent Clinical Social Worker (LICSW) in the state of Washington.

• A minimum of two (2) years of relevant post-graduate clinical experience, particularly in supervised addiction medicine therapy, within the last five (5) years.

Compensation and Benefits\n
