Key Responsibilities\n

• Evaluate and diagnose a wide range of psychosocial issues, applying advanced clinical social work skills to help patients and their families navigate personal and environmental challenges linked to mental or physical illnesses.

• Develop comprehensive treatment plans using individual and group therapy modalities, ensuring each patient receives tailored interventions based on evidence-based practices.

• Conduct bio-psychosocial assessments to identify barriers to care and enhance overall patient health, improving resource utilization across the board.

• Maintain thorough and accurate records of social work activities in accordance with departmental policies, documenting patient progress and care objectives.

• Educate patients and their families on navigating medical recommendations, fostering understanding and compliance to restore optimum health and psychosocial functioning.

• Consult with medical and support staff to design integrated care plans, aimed at increasing patient satisfaction and treatment adherence.

• Coordinate advocacy efforts in the community to ensure patients receive necessary support and resources, emphasizing self-determination and social integration.

• Participate in ongoing professional development through training and educational opportunities, as well as staying informed about community resources relevant to patient care.

• Adhere strictly to the National Association of Social Workers (NASW) Code of Ethics and departmental standards to ensure the highest quality of care in a safe environment.

Qualifications\n
