Key Responsibilities\n

• Conduct comprehensive assessments of clients to develop individualized treatment plans. This includes creating crisis and behavioral guidelines, as well as medication treatment plans as necessary.

• Ensure that all documentation meets regulatory standards and is completed in a timely manner, contributing to the efficiency of our clinical operations.

• Utilize evidence-based screening tools and best practices to monitor patient progress, making adjustments to treatment plans based on clinical judgment and patient needs.

• Collaborate with interdisciplinary teams to establish and implement positive behavior support plans, ensuring cohesive care for each client.
\n
• Serve as a liaison between clients and relevant service systems, advocating for their needs and connecting them with community resources.
\n
• Engage in ongoing professional development opportunities, including attending relevant workshops, conferences, and statewide meetings to enhance skills and representation of the agency.

• Promote and uphold the mission and values of Transformative Unicare within the community by establishing connections with local organizations and potential funding sources.

• Participate actively in licensing and accreditation processes, ensuring compliance and high standards within the clinic.

• Perform additional related duties as assigned by the Clinic Director that align with organizational objectives.

Patient Population / Client Focus\n

You will work with a diverse clientele who may be facing challenges related to mental health, substance use, or developmental disabilities. Our comprehensive approach ensures that each individual receives personalized care, focusing on their unique experiences and recovery journeys. Our clientele may include adults and adolescents, necessitating a counselor adept in addressing a variety of mental health concerns while tailoring strategies to meet their specific needs.

Qualifications\n

• Master’s Degree in a behavioral health field is required, along with an independent LMHC license issued by the state board.

Compensation and Benefits\n
