Alliance Health is seeking a Supervisor II Facility-Based Care Managers for a full-time remote role based in North Carolina (Morrisville area). The team provides transitional care management for members in acute, state-operated facilities, and justice system settings. The candidate must reside in NC and travel to offices for meetings as needed. The position requires either a registered nurse license with 5 years of experience in relevant settings or a Master’s in human services with 3 years #J-18808-Ljbffr Alliance Health
