Key Responsibilities\n

• Conduct comprehensive psychosocial assessments for individuals, families, adolescents, and groups, understanding their unique challenges and developing tailored treatment plans.

• Facilitate initial patient assessments and triage care appropriately, ensuring timely and effective intervention in treatment plans.

• Document patient interactions and treatment plans clearly while participating in interdisciplinary team meetings or patient rounds to discuss ongoing patient care progress.

• Provide case management, combining your expertise with collaboration among healthcare providers to ensure that patients access the full range of resources they may need.

• Utilize evidence-based practices, including Cognitive Behavioral Therapy (CBT), to develop and implement individualized service plans aimed at enhancing the mental well-being of clients.

• Carry out timely and ethical documentation as per department standards, maintaining accurate records of patient care and progress.
