Key Responsibilities\n

• Collaborate with primary care providers to address mental health and psychosocial challenges faced by patients, contributing as a vital member of the healthcare team.

• Conduct comprehensive patient intakes, focusing on diagnostic evaluations and functional assessments, making detailed treatment recommendations to primary care providers.

• Deliver brief, effective interventions tailored to the needs of clients requiring immediate mental health support.

• Evaluate which patients may benefit from ongoing mental health care versus those who should be referred to specialized facilities, working closely to ensure continuity of care.

• Follow up with patients to assess their progress and determine if modifications in treatment plans are necessary to meet their evolving needs.

• Design and implement relapse prevention plans, assisting patients in maintaining stability and achieving their personal goals.

• Identify at-risk individuals early and develop proactive strategies to mitigate further deterioration of their mental health.

• Coordinate behavioral health services seamlessly with other healthcare providers, both within and outside our organization, with patient consent.

• Maintain thorough documentation of patient progress and treatment plans to inform ongoing care and facilitate communication with other providers.

• Educate patients about self-management strategies and empower them to actively participate in their recovery.

• Consult with management and colleagues regarding program evaluation and development within the behavioral health services.

• Adhere to the Code of Ethical Conduct for Clinical Professionals and engage in professional development to enhance clinical skills.

• Complete other assigned duties to support organizational objectives and patient care initiatives.

Compensation and Benefits\n

The salary range for this role is between $87,000 and $105,000 annually, commensurate with experience and qualifications.
