Work Environment

This position entails working onsite within clients' homes across Hillsborough and surrounding areas. You will be an integral part of a multi-disciplinary team, collaborating closely with healthcare professionals to develop comprehensive care plans tailored to the needs of each patient. Your role may take you into various home environments, requiring adaptability to different settings and patient situations.

Key Responsibilities

As a Physical Therapist at Pinnacle Rehab Associates, your day-to-day responsibilities will include:

• Directing rehabilitation efforts to prevent disability and relieve pain, while improving and restoring muscle function through carefully adhered treatment plans.
• Supervising and guiding Physical Therapy Assistants to ensure the effective implementation of care plans.
• Conducting thorough evaluations of clients' physical capabilities by administering diagnostic tests to ascertain muscle, nerve, and joint function.
• Assessing home environments to identify modifications that can enhance client independence and safety.
• Delivering physical therapy services at home as prescribed by referring physicians, which may include manual therapeutic exercises, functional activities, and the application of physical agents.
• Evaluating, fitting, and adjusting prosthetic and orthotic devices, and collaborating with physicians on necessary modifications.
• Participating in interdisciplinary team meetings and case conferences to discuss client progress and care strategies.
• Documenting clinical records, including initial evaluations, progress notes, and discharge summaries, in a timely and accurate manner.
• Providing education and support to clients/families regarding physical therapy techniques and equipment use as well as overseeing the activities of Physical Therapy Assistants.
• Completing functional assessments and continually reviewing treatment plans as required by client progress.

Patient Population / Client Focus

Your work will primarily focus on a diverse range of patients, including the elderly, individuals recovering from surgeries, those with chronic pain, and patients needing rehabilitation after serious injuries or illnesses. You will empower clients by helping them regain independence and maintain quality of life within their own homes.

Qualifications

To excel in this role, candidates must meet the following qualifications:

• A Bachelor's Degree in Physical Therapy from an accredited program.
• A minimum of one year of experience as a physical therapist, preferably in a home care or acute care setting.
• Current licensure as a Physical Therapist in good standing in New Jersey.
• Current CPR certification.
• A valid driver's license and proof of automobile insurance.
• Exceptional communication, empathy, and motivational skills are critical.
• A strong commitment to clinical excellence and patient-centered care.
• Creative problem-solving abilities and a readiness to take initiative.

Preferred Experience

While not mandatory, additional experience in geriatric care, knowledge of current therapeutic modalities, and familiarity with electronic health records (EHR) systems would be advantageous.

Compensation and Benefits
