This position allows for a remote work setting, with a typical work schedule from Monday to Friday, 8:00 AM to 4:30 PM. You will also be required to maintain on-call availability to respond promptly to urgent situations that may arise after hours.

Key Responsibilities\n

• Conduct detailed assessments of the psychosocial needs of seniors, using various assessment tools to evaluate emotional, mental, and social factors that affect their lives.

• Collaborate with clients and their families to create personalized care plans that address individual goals, preferences, and challenges, promoting a strengths-based approach.

• Provide counseling and emotional support, facilitating discussions on life transitions such as health changes, loss, and adjustment to hospice care.

• Actively advocate for clients' rights, ensuring they have access to essential community resources, healthcare services, and support systems.

• Work closely with interdisciplinary teams, including nurses, caregivers, and other healthcare professionals, to coordinate effective care tailored to each senior's needs.

• Conduct regular home visits to monitor client well-being, assess living conditions, and proactively address any safety or health concerns.

• Facilitate clear communication among family members involved in making care decisions, and help to resolve any conflicts that may arise in the caregiving process.

• Educate clients and their families about community resources, support groups, and other programs that can foster social engagement and improve overall quality of life.

• Maintain meticulous documentation through electronic health record (EHR) systems, ensuring that assessments, care plans, and progress notes are up-to-date and compliant with regulatory standards.

• Stay informed about current best practices in social work and hospice care, continually enhancing your knowledge to improve client outcomes.

Qualifications\n

• A current and valid license to practice as a Clinical Social Worker (LCSW) in Texas.

Compensation and Benefits\n

This position offers a competitive annual salary ranging from USD 60,000 to 100,000, commensurate with experience and qualifications.
