Key Responsibilities\n

• Conduct comprehensive evaluations and assessments for children in the hospice care system, ensuring accurate diagnosis and tailored support.

• Participate actively in the development and implementation of Individualized Family Service Plans (IFSPs), ensuring they align with best practices and family priorities.

• Monitor and document outcomes to assess child and family progress as a vital member of the care team.

• Provide emotional and practical support to families, equipping them with tools and strategies to enhance their child's development.

• Ensure compliance with all relevant federal and state regulations governing mental health and social services.

• Foster strong relationships with children, families, and team members to create a positive and supportive learning environment.

• Adopt evidence-based practices within the frameworks of Natural Learning Environment Practices to maximize child engagement.

• Implement IFSPs and recommend service frequency based on ongoing assessments of child development status.

• Utilize a coaching model and effective teaching techniques within natural environments to support family engagement.

Patient Population / Client Focus\n

This role primarily focuses on serving infants and toddlers within hospice care who exhibit developmental delays or disabilities. Qualifications\n

Compensation and Benefits\n
