Key Responsibilities

• Function autonomously and collaboratively to deliver comprehensive care for patients with complex, acute, chronic, and critical health conditions.
• Prescribe and manage medications, carefully determining dosage and frequency based on evidence-based practices and individual patient needs.
• Engage in interdisciplinary teamwork, consulting with other healthcare professionals to enhance patient care quality.
• Facilitate patient transitions effectively, ensuring clear communication among healthcare providers and continuity of care.
• Document and maintain accurate patient records in accordance with legal and regulatory requirements, promoting efficient access to health information.
• Educate and advise patients on managing their health conditions, developing tailored treatment plans, and recommending further care as necessary.

Patient Population / Client Focus

The Nurse Practitioner will primarily serve a diverse patient population, including individuals from various cultural backgrounds, with a special emphasis on providing care in both English and Spanish. Qualifications

• Current Nurse Practitioner certification from a recognized institution.
• Active nursing license in the state of Texas.

Compensation and Benefits
