Work Environment

This position is primarily on-site within our Missouri City facility, allowing you to engage directly with patients in their homes and community settings. Your work schedule will typically span from 9:00 a.m. to 6:00 p.m., with occasional extended hours for evening clinics until 8:00 p.m. Participation in community health fairs and outreach events during weekends may also be part of your responsibilities, solidifying our commitment to community health and engagement.

Key Responsibilities

• Conduct thorough health assessments and screenings of patients, creating tailored care plans based on comprehensive evaluations.
• Diagnose and manage both acute and chronic medical conditions, ensuring preventive care and health promotion are at the forefront of your practice.
• Work closely with community organizations to develop and implement effective health education programs designed to raise awareness about important health issues.
• Deliver culturally sensitive care and advocate fervently for patients' rights and their access to necessary healthcare services.
• Coordinate patient referrals and manage follow-up care to guarantee continuity and effectiveness in treatment.
• Document patient interactions and treatments accurately to maintain compliance with all regulatory standards, ensuring a high-quality care delivery process.
• Engage in community outreach initiatives, providing health screenings and education to further health awareness among the public.
• Stay informed on community health trends, evidence-based practices, and evolving policies to continually refine your practice.

Qualifications

• A valid Nurse Practitioner certification and active state nursing license are required to practice in Texas.
• A Master’s degree in Nursing from an accredited institution is necessary for this role.

Compensation and Benefits

We offer a competitive salary range of $120,000 - $130,000 per year, commensurate with experience, alongside a comprehensive benefits package that includes medical, dental, and vision insurance, a retirement plan, and paid time off.
