Outpatient Physical Therapist

Overview Tufts Medical Center is seeking a dedicated Outpatient Physical Therapist to join our Healthcare & Medical Services department in our busy, patient-centered outpatient practice. As a key member of a collaborative, interdisciplinary team, you will provide evidence-based evaluation and treatment to a diverse adult and adolescent population presenting with musculoskeletal, neurologic, post-surgical, and chronic conditions. Responsibilities Independently manage a caseload of patients, performing comprehensive assessments, developing individualized plans of care, and delivering manual therapy, therapeutic exercise, neuromuscular re-education, and functional training designed to restore mobility, reduce pain, and improve quality of life. Regularly measure outcomes, adjust treatment plans based on patient progress and current best evidence, and communicate clearly with referring providers, case managers, and other members of the care team. Document thoroughly and timely in the electronic medical record, ensuring compliance with hospital, state, and federal regulations and payer requirements. Provide clear home exercise programs, self-management strategies, and injury-prevention guidance tailored to each patient’s goals, culture, and health literacy level. Participate in student education, mentoring, in-service training, and quality‑improvement or research initiatives that align with Tufts Medical Center’s mission. Collaborate closely with physicians, nursing, occupational therapy, and other allied health professionals to coordinate care and optimize patient outcomes across the continuum. Qualifications Patient-focused, adaptable, and committed to continuous learning, evidence-based practice, and cultural humility. Comfortable working in a fast-paced outpatient environment with strong clinical reasoning and clear communication skills. Experience with orthopedic and sports rehabilitation, post-operative care, or chronic pain management is highly valued.
